EV Charger Attestation Form
This form requires the applicant to confirm they understand the program requirement to install a level 2 EV charger at their home within 180 days of receiving the electrical panel upgrade rebate.

Household Summary Form
This form collects information to determine if an applicant qualifies for the income-based electrical panel upgrade rebate based on household size and income limits.

Proof of Income Eligibility
The 4506-C is an IRS form we use to obtain a copy of your tax return transcript for income verification. A 4506-C must be submitted for each person over the age of 18 included in your tax return for the previous year. Digital signatures on this form will not be accepted. If you encounter any issues covered under warranty, please follow these steps to file a claim:

- Alternatively, you can email us at evhome@scerebate.com with your project details, a description of the issue, and any relevant documentation.
- Initial Assessment:
- Our customer service team will acknowledge your claim within 1 business day and may request additional information.
- Resolution:
- If the issue is covered under warranty, we will provide a hand-off with the contractor.
- If the contractor does not provide a resolution, they will risk being removed from the program.
